This 30 minutes talk will focus on how senior leaders are fooled by randomness when it comes to managing safety. Based on a real life case study, Nippin will demonstrate why focusing on accidents i.e. undesirable, unexpected outcomes can become a dangerously misleading indicator of safety. To a large extent, the issues arise due to the available accident investigation and analysis models, methods and approaches (focused on outcome). Understanding these issues and becoming aware of their limitations sets us on a path towards learning and improvement.
